For access to your medical records, we now utilize the services a medical records company, Chartfast.  You are now able to request your records electronically by simply creating a requester account at www.chartfast.com. 

Once the request is fulfilled, you will receive an email that your records are ready for download!  This site also allows you to pay for your records electronically.  If you would like to check the status of your request, you may log into the account that you have created and all of this information will be readily available. 

While accessing your medical record via Chartfast is the most secure method of receiving your protected health information, we understand that this may not be right for you.  For alternate request and delivery methods please use;
